Physical Form May 12, 2010

Please turn this into your child’s coach or Mr. Ehrbar at the middle school. I highly recommend taking care of this over the summer.

Mayfield Middle School
Eligibility
Eligibility requirements established by the Ohio High School Athletic Association state that: To be eligible, a student-athlete must be currently enrolled in a member school and have received passing grades in 75 percent of the subjects in which enrolled during the immediately preceding grading period.
For eligibility purposes, summer school and/or night school grades may not be used to substitute for failing grades received in the final grading period of the regular school year or for lack of enough courses taken the preceding grading period.
Special Note: Board Policy
In addition to the eligibility requirements established by the Ohio High School Athletic Association, to be fully eligible to participate in interscholastic athletics and District- sponsored clubs and activities, a student enrolled in grades 7 through 12 must:
ü Achieve the minimum grade point average (1.50) in the nine- week period preceding participation.
ü Receive no failing grades in the nine-week period preceding the period of his/her participation.
A special conditional eligibility status may be granted by the principal if the student has received only one failing grade in the previous nine (9) weeks and maintains the required GPA level. This conditional eligibility status requires the student with one failing grade to meet a series of weekly requirements to maintain a conditionally-eligible status.
Equipment
Once equipment is issued, it is the player’s responsibility. Equipment must be turned in on time. Report cards will be held and students will be charged for unreturned uniforms.
Transportation
All athletes will travel with the team. The only exception would be the release of the athlete to parents for a family activity (a parent request must be made in writing). However, parents are discouraged from making this request.
Medical
You must have a physical form on file in the Athletic Office. A physical must be done yearly.
Admission
All middle school contests in a self-contained facility (gym or enclosed stadium) will be charging admission.
Adults - $2
Students - $1
Playing for teams (same sport) other than Mayfield
A member of an interscholastic squad sponsored by the Board of Education shall not participate in a contest, tryouts, or any type of training or practices on a non-interscholastic squad or as an individual in a team sport, in the same sport during the school’s interscholastic sports season.
Double Headers
Seventh grade plays first in games that fall on a Monday or Tuesday.
Eighth grade plays first in games that fall on a Wednesday, Thursday, or Friday.
